Any personal identifiable information requested by IQH, i.e., contact name, address, phone or fax number, or email address, is protected by the IQH confidentiality policy. This information is used solely by IQH for the information or service that is requested. IQH will protect the personal information shared. IQH does not disclose, give, sell, or transfer any personal information to third parties.

Electronic mail is not secure against interception. If any communication is confidential or sensitive, it should be sent by U. S. mail instead. Confidential information, Social Security numbers, Medicare numbers, patient or provider identifiable Protected Healthcare Information (PHI), etc., should not be sent to IQH via email or through this website.

Website Policies

IQH’s website automatically collects and records the following information about each visitor: date and time of visit, pages visited on the site, search engine and key words used to find the site, type of computer and browser the visitor is using, and the location of the visitor’s machine by country, not street address. This information is used to improve the service for the visitor and is not shared with third parties.

CME Privacy and Confidentiality Policy

Information & Quality Healthcare (IQH) respects and values the privacy and confidentiality of information shared. IQH is committed to protection of this information and will assure visitors’ privacy when participating in CME activities on the website.

When a person enrolls in an IQH-hosted course offering, personal information collected is for the purpose of processing the registration and awarding CME credit. IQH serves as hosting agent fo CME testing and will provide the information collected to the organization that awards the CME credit.
